The story offers little detail on Sarah's looks beyond the note that the 2003 anime depicts her as a brunette. That same version has Van Hohenheim remark on how closely Winry resembles her, twice mistaking the daughter for the mother.

Sarah is portrayed as a determined and strong-willed woman, driven above all by the wish to help and save those in need. Offered the chance to leave Ishval with the other Amestrian civilians, she refuses, telling Yuriy that Winry and Pinako would never forgive her for abandoning her calling and reminding him that Rockbell women are made of stern stuff, even though she joined the family only by marriage.

Sarah earned her living as a surgeon and married Yuriy Rockbell, the son of Pinako Rockbell, practicing medicine at his side. When the Ishval conflict reached its height, the couple left their Resembool home to tend the wounded at the front. Amestrian by birth, they nonetheless became well regarded across the Kanda region of Ishval for treating casualties on both sides of the fighting, and figures such as Doctors Marcoh and Knox looked on them as a medical inspiration. The military, however, came to view their humanitarian work as an obstacle and moved to have them killed.

In the manga, Solf J. Kimblee is quietly tasked with staging their deaths as an accident, but before he can act the pair are slain by Scar, who has just awakened in their aid station and been thrown into a murderous rage on realizing his brother is dead; Yuriy throws himself before Sarah in a futile bid to shield her. The 2003 anime alters the circumstances, having Basque Grand formally order their execution as insurgents because their treated Ishvalan patients would rejoin the fight, and Roy Mustang is made to carry out the killing against his conscience. In that telling, Winry learns of her parents' fate from Rick and Leo, two Ishvalan children whose mother had received medicine at the Rockbell clinic.
