Polite one moment and savage the next, Misawa has battered a downed flight attendant with the stock of her shotgun and, in the novel, is shown to relish inflicting pain both physical and emotional. She claims her first kill at fifteen, bristles at children, and says she cannot stand the sound of their voices. On film she first appears tending plants, then stamps on the corpse of a man Matoba has just stabbed, musing that everyone dies in the end. At the research lab she charms a night guard before knifing him and cuts down a worker, meant to seize Maki Nikaido as leverage until the professor takes his own life. For much of the movie she supplies muscle, surfacing late to march L at gunpoint toward K. When L urges K to release the antidote, Misawa instead snatches an infant from a passenger and threatens to kill it, only to collapse as the virus sweeps the cabin. She is arrested alongside Kujo once the crisis passes.

The novel expands her cruelty. She gleefully produces Maki to break Professor Nikaido, snaps at the girl to be quiet, and later injects Maki with the virus to force L's hand. She taunts the captured Suruga, shrugs off Kujo's anger, and stays unmoved when Yoshizawa is shot dead, recalling only that the two of them once made a capable pair. Aboard the Los Angeles flight she pries open an emergency door out of boredom, and when Konishi hesitates to shoot her, she kicks away his gun and buries her umbrella-shiv in his throat, scolding him for flinching. She climbs onto the wing, where L wrests away the stiletto she has carried throughout and drives it into her foot, sending her tumbling from the aircraft. Her ultimate fate is left unresolved.
