Kawaki is a boy raised and experimented on by the organization Kara, groomed for years to serve as the vessel of Isshiki Otsutsuki. In calmer moments he proves sharp and pragmatic, reading a fight as it unfolds, seeing through deceptions, and improvising cunning plans. The years Kara spent forging him into a weapon scarred him so deeply that something as small as dripping plant roots can spark a panic attack, and he grew to despise what he had been made into. He nurses fierce grudges, above all against Jigen, whom he blames for betraying a promise of safety, and by extension he loathes the whole Otsutsuki clan.

Life with Naruto's household slowly reshaped him. He warms to games, grows curious about ninjutsu, and comes to revere Naruto as a father figure after learning how alike their lonely childhoods had been, later embracing Boruto as a brother. That loyalty runs so deep that he would sooner die than live in a world without Naruto, and he steps into another dimension to fight at his side. It also carries a darker edge: his devotion does not reach others, and once he inherits the means to erase every Otsutsuki, he resolves to kill Boruto for Naruto's sake, fully aware the choice is monstrous and braced to be hated, even killed, for it.

Born to a violent, drunken father named Kokatsu, Kawaki was sold to Jigen of Kara for a large sum. Jigen brought him to Amado's laboratory and branded him with Isshiki Otsutsuki's Kama; of fifteen candidate children, only Kawaki endured the transfer, waking with the mark on his hand and, from then on, a life he would describe as a living hell. Reshaped into an Otsutsuki-styled cyborg through implanted Shinobi-Ware and drilled without mercy by Jigen, he was meant to become Isshiki's flawless body.

When a test run to the surface went wrong, Kawaki fled and collapsed in the Land of Fire, where Team 7 found him bearing the same seal as Boruto. After a clash with the Kara agent Garo, he was taken to Konoha, and the Five Kage agreed he would live under Naruto's watch. Bonds with the Uzumaki family softened him even as Kara closed in; Naruto defeated Delta, gave Kawaki a prosthetic arm, and began teaching him ninjutsu. Jigen soon abducted him and revived Isshiki, and though the crisis ended in Isshiki's death, Amado restored Kawaki's Kama as a pure weapon, steadily rewriting him into a complete Otsutsuki with access to Isshiki's Kokugan and its shinjutsu.

Fixated on shielding Naruto, Kawaki turned on Boruto, and the reality-bending force of Ada's Omnipotence swapped the two brothers' places in the world's memory, leaving nearly everyone to know him as Kawaki Uzumaki. He banished Naruto and Hinata to another dimension and framed Boruto for the Hokage's supposed death. Three years later, in the Two Blue Vortex era, Amado's adjustments let him trade blows with Code and the God Trees, while his private war with Boruto simmers toward a final reckoning.
