Because Reiko made her name in gaming, her Phage form looks pulled straight out of a video game: blue skin, white hair, and fangs, with a pair of bat-like wings and a red-and-black costume trimmed in pink.

The trouble starts after Ami wins a gaming event that Taiki is co-hosting with Reiko. The two slip into a back room to talk, only to hear Reiko screaming as Sailor Lead Crow attacks her. Ami becomes Sailor Mercury to face the new Phage while Taiki simply walks away, telling himself the planet is not his concern. Mercury is badly outmatched until Sailor Jupiter arrives, along with Minako and Rei, who are still in their event cosplay rather than their guardian forms.

Even the four of them struggle, and the Sailor Starlights only turn up after Seiya talks them into helping. The inner guardians refuse to let the Starlights kill the Phage and hold the line themselves until Eternal Sailor Moon arrives to heal her with the Silver Moon Crystal Power Kiss.

Sailor Gamer comes loaded with a video-game-style move list, including her Forest Wind Attack, Stormy Blizzard Attack, Vanishing Bomb Kick, and Deadly Attack Sonic Beam. Her bat-winged look ties her to earlier monsters of the same mold, the Youma called Kyurene and the vampire Lemures, and she stands as the last female monster the anime ever produced.
