
Heizō Kasaki was the third seat of the Gotei 13's Ninth Division, serving under Captain Kensei Muguruma. A steady, level-headed officer, he appears in the Turn Back the Pendulum flashback and is cut down during the mission that would later transform his captain into a Visored.
Tall and heavily built, Kasaki is a bald Shinigami who sports a mustache and a habitually stern expression. Over the usual division garb he adds a high-collared white jacket in the style of the Ninth Division.
Kasaki is disciplined and even-tempered, holding his composure where others would snap. When Lieutenant Mashiro Kuna's ceaseless complaining on a mission drove Kensei toward striking her, Kasaki was the one working to defuse the situation. His captain also trusted his organizational sense, handing him the task of setting up camp during a critical operation.

Kasaki joined Kensei and the rest of the Ninth Division's officers to look into a rash of disappearances among the Rukongai population. As Mashiro's griping wore on Kensei's patience, Kasaki bit his tongue until his captain finally lunged at her, then helped the others hold Kensei back. The unit also rescued the young Shūhei Hisagi along with his companions from a stray Hollow. When Mashiro turned up ten empty uniforms near the attack site, Kensei connected them to a missing scouting party.
Treating the case as far graver now that Shinigami had vanished, Kensei issued orders and put Kasaki in charge of bringing up the standby unit to pitch camp. That night, while Kasaki rested alongside his captain and lieutenant and the other officers stood watch outside, screams rang out. He and Kensei rushed into the open to find their comrades slain, drew their blades, and were set upon at once. Kasaki was killed in the ambush, and Kensei was left gravely wounded. As a third-seat officer he was a competent swordsman, though the powers of his Zanpakutō were never revealed.
