Barbara carries the ordinary frame of a working mother, her dark hair kept in a short bob. Off the clock she favors a pale headband paired with an Aloha-print tee, while her airport uniform swaps that out for a sunhat and a light polka-dotted dress.

Being born into the Joestar line, she is expected to carry the family's telltale star-shaped birthmark, though the mark has not yet been shown on her.

Routine governs Barbara's mornings. She wakes before 7:15, immediately starts the washing machine, tidies herself in the bathroom, then cooks a proper breakfast of ham and eggs, the eggs either fried or given a seven-minute boil, rounded out with toast, fruit, and cooked broccoli. She kisses her sons goodbye, hangs the laundry to dry, and still makes the 8:06 bus to her duty-free counter at the airport.

Because Jodio and Dragona operate in the criminal underworld, they make certain their mother is shown constant deference. One neighborhood child carries her bag each morning, and when it rains another walks her to the bus stop under an umbrella.

Barbara and her husband relocated from Atlantic City to Honolulu on Oahu while Jodio and Dragona were eleven and fourteen, and around that move the couple's bond started to fray. After Jodio set a fire to shield his brother from bullies, she spoke with her husband, relieved that Dragona had skipped school that day yet troubled by Jodio's falling grades. Months on, she explained to the boys that their father had been pressured into paying ten times the standard rate to insure the bus company, then dismissed for agreeing, which drove him out of Honolulu and left the marriage broken.

Raising the two alone, Barbara took a duty-free post plus a late-night job, and the strain wore on her health as debts mounted and gangs spread through the neighborhood. Meryl Mei Qi recalls that she could not cover the school tuition. A visit to Jodio's school counseling once required her to pay for a psychological evaluation, and when it rated him with eighteen of the twenty markers of a psychopath, she erupted and demanded a refund.

In the present, she phones Jodio to ask where he and Dragona have gone after failing to reach Dragona directly, wonders aloud what the family wants for dinner, and warns her son away from Paco Laburantes, whom she considers a bad influence.
