Tenko drifts through the streets after his family dies, invisible to everyone except one woman who takes in his ruined face and suggests a hero or an officer will turn up eventually. He treats the neglect as his sentence and wonders whether a single offered hand would have quieted the itch. All For One offers that hand, tells him the craving to destroy is simply who he is, and lets it run. When two thugs corner the boy, the urge wins. That night, wearing his relatives' hands, he kills them both with Decay. Doctor Ujiko notes the sealed memories keeping him unstable and holding his Quirk below its ceiling, which suits All For One fine. Fresh hands are gifted, and the name Tomura Shigaraki is bestowed, mourning built into the syllables.

Back in Deika, the recovered memories finish assembling. Tomura discards his father's last hand, shrugs off Stress: 100%, and Decays the energy out of the air. Re-Destro deploys Claustro, an armored rig that drives his output to 150%, and still loses the exchange. A hand to the ground spreads ruin outward through the whole city, Tomura's hair bleaching white as he laughs. Re-Destro survives only by amputating his own legs mid-fall. Trumpet's Incite dies against a single glare, and the grand commander bows, handing the Meta Liberation Army over.

A week later Skeptic's edits have reframed the disaster as brave residents repelling twenty criminals without hero support. The League convalesces at the Gunga Mountain Villa over sushi, bickering about who actually contributed. Underground, Re-Destro presents Tomura as the movement's new leader, and the merged organization takes the name Spinner helped coin: the Paranormal Liberation Front. Afterward, drained and slumped, Tomura hears Ujiko confirm that the promised power is his now that memories and Quirk have both returned.

All For One christens Tenko Shimura as Tomura Shigaraki, explaining that the given name derives from the verb to mourn.

Tomura's restored Decay levels Deika City, defeating Claustro and Stress: 100% outright. Gigantomachia weeps at the resemblance to his master and accepts Tomura as the successor.

Re-Destro concedes leadership of the Meta Liberation Army, which Tomura entertains largely because the Detnerat CEO has money the League badly needs.

The Meta Liberation Army and League of Villains merge into the Paranormal Liberation Front, with Tomura installed as Grand Commander and nine lieutenants at his side. Doctor Ujiko signs off on the name and promises the upgrade he owes.

The adaptation reshuffles a fair amount. Flashbacks of the dead family accompany Tomura's recollection, the League noticing Gigantomachia's arrival is cut, and his hair whitens later here than in print. Trumpet fleeing the Decay, Spinner tagging along, and Spinner's narration are all absent, while Skeptic's dash toward Re-Destro and Twice's rescue of Giran are moved earlier. A new beat shows Claustro's right arm decaying before the city falls, Re-Destro is toppled by shockwave rather than debris, and the Chapter 220 flashback about the League's finances is finally included. Former U.S.J. villains Victor and Chomper appear in the doctored footage, and Ujiko's closing request referencing Heroes: Rising is dropped.

During its Toonami premiere this was the only first-run season five episode to carry the full opening sequence rather than the shortened cut. Tomura laughing amid the destruction echoes the Volume 25 cover, itself drawn from a panel in Chapter 239.
