A month before the fighting, Hawks works the Paranormal Liberation Front from the inside on the Commission's behalf, holding the single clue hospital. Twice, unable to ask anyone else, begs him for help articulating the organization's credo. Hawks obliges with vague talk of individual freedom and deference to Re-Destro's bloodline, all while cataloguing the regiments the Front has organized: Twice over the Black, Dabi and Geten the Violet, Himiko and Skeptic the Carmine, Spinner and Mr. Compress the Brown. Their combined strength outclasses the professionals, and the plan is simultaneous nationwide attack followed by political capture and armed chaos with Tomura enthroned. Twice, who once let slip that Tomura is spending four months growing stronger, thanks him for being kind, and Hawks smiles back at a man he has already decided is the most dangerous person alive.

At Police HQ the heroes profile Doctor Kyudai Garaki, ostensibly a Quirkless philanthropist and hospital founder, in truth the man who engineered every Nomu at All For One's side. Photographic proof came from an undercover operative who reached his restricted floor. Rather than risk a reprisal, the profession commits everything to two coordinated strikes: Team Endeavor seizes Garaki at Jaku General, Team Edgeshot storms the Gunga Mountain Villa, with U.A. students assigned to rear-guard support and a handful posted forward for wide-range attacks.

The raids begin. Slidin' Go, the Front's informant, is arrested mid-patrol. Burnin sends the trainees into Jaku to clear civilians while Endeavor's force floods the hospital. Aizawa's Erasure strips Garaki of a Quirk he had hidden all along, Life Force, which explains All For One's longevity, and Present Mic demands to know why such a mind chose such work. Then a drill Nomu erupts through the floor and impales the doctor, revealing him as one of Twice's Doubles. Below ground the real Garaki grabs his samples and runs for Johnny, only for Mirko to smash through the wall, crush the warp Nomu, and ask whether this one is genuine.

Hawks maps the Paranormal Liberation Front's regimental structure and its plan to attack every city at once, identifying Twice's Double as the single greatest threat now that his trauma is gone.

Naomasa Tsukauchi exposes Kyudai Garaki as the mind behind the Nomu and All For One's long-serving accomplice, prompting a two-front operation against the hospital and the villa.

Eraser Head's Erasure ages Garaki drastically, revealing the concealed Life Force Quirk. Naomasa theorizes the doctor also reproduced Super Regeneration artificially for the black Nomu.

The captured Garaki turns out to be a Twice Double, which vanishes only after noting that Erasure does not dispel copies. Nomu pour into the hospital, and Mirko breaches the secret laboratory, killing Johnny and destroying several High-End containment canisters.

The sixth season premiere adapts Chapter 258 through Chapter 260, introducing the opening Single-Minded and the ending SKETCH.

Additions and changes are numerous. Majestic joins the villa front line, Midnight holds Kinoko's hands while explaining the students' placement, a scene explains why Denki, Fumikage, Kinoko, and Juzo were posted forward, Natsuo and Fuyumi worry about their father after leaving home, Police HQ monitors both raids, and the Nomu assault is expanded. Where the manga pictured a mix of Twice and Hood Doubles during Hawks' assessment, the anime shows only an army of Twices.

A cameo by Denji from Chainsaw Man among the villa raid team was swapped for an unnamed saw-headed hero, most likely because a rival studio's Chainsaw Man adaptation was airing concurrently. The English dub gives Mirko a Bugs Bunny nod when she breaks into the laboratory.
