Psykos presents as a striking young woman with a curvy build, waist-length wavy hair in a pale aquamarine shade split down the center, and green eyes usually framed by black glasses. Cunning and manipulative in equal measure, Psykos plots endlessly among the Monster Association's ranks to advance her experimental goals, going so far as to feed Rhino Wrestler into hopeless matchups purely to satisfy her curiosity. Arrogant, cruel, and prone to sadistic games with the Hero Association's staff, she coldly weighs the compatibility of every battle and shows no mercy toward subordinates who disappoint her.

Her ego is fed by her esper power and by the extra strength granted to her by the entity she calls God, which convinces her that she is meant to reign as the ultimate lifeform and leads her to look down on Tatsumaki. Opposition, whether from Tatsumaki or from Fubuki, sends her into a fury. Her grudge against Fubuki burns especially hot, rooted in a shared past that hints at deeper feelings during their youth; those feelings curdled into rage when Fubuki declined to help her wipe out humanity and sealed away her powers for a time.

Fubuki later explains that something broke inside Psykos and drove her to madness. A vision of the future she once witnessed turned her toward hatred of humankind and inspired her to build the Monster Association. Before that vision consumed her, she had been a clumsy person who deeply admired her old friend.

She can beam psychic signals to allies across vast distances and pilot Gyoro Gyoro from 1,500 meters underground, fire energy from her eyes, warp gravity to crush foes, and deploy drones shaped like her puppet as flying spies or fixed cameras. Telepathy lets her converse remotely, while her Third Eye grants glimpses of the future, the very ability whose vision pushed her toward madness.

Beyond her powers, Psykos is a brilliant strategist who grades threat levels within the Monster Association and pairs monsters against heroes by battle compatibility, engineering lopsided fights that punished many high-ranking heroes. She also claims she can spur ordinary humans into monstrous growth, as she did with Orochi, and she keeps an Elixir that amplifies her psychic strength, which she downs before challenging Tatsumaki.

After Orochi barely survives Saitama, he absorbs Psykos to rebuild himself, but she resists total assimilation and the two combine into a colossal, tentacled entity that encounters God and receives yet more power. The fused being contends with Tatsumaki, hurls attacks fierce enough to raise tsunamis across part of the planet, and can drain the powers of others while anchoring itself to the earth through roots. It conjures psychic decoys, reshapes its horns into a giant avatar of Psykos, extends its fingers into massive spears, unleashes electricity and rapid regeneration, spews smoke for cover, and even reshapes into a jet to fly. Psykos muses that the amalgamation might one day fuse with the planet itself and awaken as a living world, though whether that outcome was ever truly possible stays unknown. She is eventually brought down, with Fubuki playing a decisive part in her defeat.
