True to her name, Twin Tail wears her hair in pigtails, each bound by four dice that all show the number four on every face, a figure that in Japanese echoes the word for death. She carries herself in the costume of a jester, a blindfold covering her eyes and a single painted teardrop on her cheek whose meaning is never explained.

Her inner life remains largely a mystery, but the impression she gives is one of assurance edged with a certain grimness. That confident, faintly morbid air suits a hero who fights blind and treats lethal blades like carnival props.

Twin Tail joins the support team assembled for the Monster Association raid, and the moment the heroes push into the abandoned district she reads the breathing of the creatures hidden around them and warns the group that they are surrounded. Her Death Loop, an accelerated flurry of thrown weaponry, cuts down several monsters, and when a Monster Association sniper nearly finishes her, One Shotter pulls her out of danger. Rhino Wrestler's arrival tests her again, and she is among the many hurled back, her strike dismissed with a rating of nine out of a hundred. After Atomic Samurai slays Rhino Wrestler, she holds the surface with the rest of the support squad while the strike team pushes into the enemy base, only to be struck down together with several others by the executive Nyan, whose speed no one could follow. Sekingar carries the fallen heroes clear, Twin Tail among them, and she is still breathing by the time Drive Knight finishes Nyan off.

She resurfaces during the Psychic Sisters Arc at the Hero Association headquarters in A-City, arriving on the lower floors after the alarm sounds to look into the disturbance caused by Tatsumaki's clash with Apollo. Her combat rests on drastically sharpened senses that compensate for the blindfold, letting her hear monsters breathe from every direction and juggle her tools without a glance, backed by the strength and speed expected of her rank. That juggling style feeds off a hidden arsenal of more than thirty concealed implements, among them quick-drawn pins dense enough to fracture bone, balls solid enough to crack a skull, and scimitars tucked into the ruffles of her collar.
