Slim in build and fond of revealing outfits, her anime design dresses her in a black corset gown with sheer sleeves and neckline, a ruffled skirt, and matching shoes, showing a full, curvy figure. As a younger woman her hair fell only to her chin, and she leaned on daring clothing to throw opponents off balance. For her ghoul disguise she dons a clown mask modeled on a carnival face, noseless and locked in a wide, toothy grin. Its right eye is shaped as a three-dotted heart and its left as a clover, spades flank the pair, and the chin juts outward.
On the surface Itori is warm, talkative, and playful, quick with a joke or a prank, yet she handles people like toys she can wring information out of, an outgoing and forceful sort. When her Clown allegiance surfaces, a nastier, crueler streak emerges; she delights openly in the CCG's fresh ambition to exterminate every ghoul. As Clowns do, she feeds on turmoil and wreckage, and her appetite for blood suggests a refined palate close to Shuu Tsukiyama's.
In the era when Uta headed the 4th Ward, Itori belonged to his crew and sometimes lured investigators into ambushes. Yomo's arrival cemented a friendship among the three that would last for years. She debuts at the Helter Skelter bar beside Uta and Yomo, immediately taken with Kaneki: she tips wine over him and pulls off his eyepatch to catch sight of his single kakugan. Floating the rumor that someone had intentionally toppled the steel beams onto Kaneki and Rize, she dangles a deal, intel drawn from the Gourmet about the Ghoul Restaurant in exchange for what she knows of that accident.
Meeting her again to recap his fruitless visit to the Restaurant and ask after Rize, Kaneki finds her irritated at his empty hands yet pleased he tried; she lets slip that Rize's given and family names were both invented, that the woman had surfaced one day having discarded her real identity, and nudges him to search elsewhere. Later he brings a roster of Cochlea escapees hoping to trade for anything on Kanou, and she happily notes his new respect for information brokering, offering only a guess that he had once been a coroner in the CCG's employ before rounding on him for wrecking the very ghoul restaurant she'd been looking into. Nico startles Kaneki by emerging from the restroom, though she waves him off as a harmless friend fixated on men. When the Owl Suppression ends, she is among the Clowns raising a toast.
Her Clown ties come to the fore when she convenes with them inside the office belonging to CCG director Kichimura Washuu and reveals that Goat has rooted its base in the 24th Ward underground, crediting the tip to Roma, whom she calls her protege. Through the Dragon crisis she hauls an unconscious Yomo to Goat's remnants and proposes they merely watch Kaneki's kakuja form be destroyed; when Goat instead resolves to rescue him, she leaves disappointed. She resurfaces alongside Uta, Donato Porpora, and Nico to watch the CCG take on V, and, meaning to stall for time, they unleash an Owl-shaped kakuja on the investigators. Rapt as Uta and Yomo trade blows, she remarks how far Yomo has drifted since Anteiku and reflects that humanity is the only kind the world will ever embrace, leaving ghouls to a permanent chill; when Yomo wins she just sighs that he alone could ever change Uta. Long after, she, Uta, and Nico go on periodically trying to kill Yomo while their friendship endures, and Uta's antics remain her private delight, feelings she may keep to herself forever.
