Kasuka Mado was a gifted Associate Special Class investigator, the wife of Kureo Mado and the mother of Akira. Ordered to hold the One-Eyed Owl at bay so her comrades could withdraw, she died on that mission, and her loss reshaped the lives of both her husband and daughter.
Her face is never fully revealed, though the resemblance to her daughter Akira is strongly implied, the main difference being that Kasuka wore her hair flat. Like the rest of the CCG, she appeared in the organization's formal dress.
Little of her nature is shown, but she came across as courageous and firm of will, squaring up to the One-Eyed Owl by herself. She also clearly held deep affection for both Kureo and Akira.
Kureo had married into her family, who happened to be his professor, after she first refused his proposal because she would not give up her surname. Both served the CCG, and ten years before the series they joined a 24th Ward operation under Itsuki Marude. Deep inside, on the 124th floor, the squad ran into the One-Eyed Owl. With casualties mounting and victory unlikely, Marude ordered Kasuka, the strongest of the team, to stall the creature alone while the others pulled back. She was slain and devoured, her scattered remains later recovered by her husband. Akira remembered her as an investigator of real strength and skill who reached high rank while still young, and her death struck Kureo and Akira hard, fueling Kureo's hatred of every ghoul.

Naruto vs Luffy with the twist nobody asked for: Luffy tickled Hinata on date night, and she kind of liked it. We break down the whole fight and commit to a winner....

There are no women on Namek. That isn't an oversight, it's the biology. Why we handed Piccolo the one thing his own species rules out, and why it had to be trap soul....
Kasuka Mado was killed and devoured by the One-Eyed Owl during a CCG operation in the 24th ward, ten years before the main story.
Kasuka Mado married Kureo Mado, a fellow CCG investigator who had originally been her professor and proposed to her only after she agreed to let her keep her own surname.
Kasuka Mado died because Itsuki Marude, recognizing her as the strongest fighter on the team, ordered her to hold off the One-Eyed Owl alone on the 124th floor so the rest of the squad could retreat safely.
Kasuka Mado held the rank of Associate Special Class investigator in the CCG, working as both a ghoul investigator and an academy instructor before her death.
Kasuka Mado's death devastated her husband Kureo and daughter Akira Mado, fueling Kureo's deep hatred of ghouls and leaving Akira with only fragmented memories of her mother's strength and skill.
