Respect colors how he treats Sasaki, Dr. Shiba, and others, even those who look down on him, and in time he grows bolder and works to pull his team together.
Beneath that gentleness sits a fractured self. Mutsuki endures blackout episodes that carve gaps into his memory, and during them a cold, violent side takes hold, leaving him with no recollection afterward and quick to deny what he has done. When the truth of those lost hours reaches him, he recoils in horror. His stability crumbles further as an obsessive love for Sasaki sours into cruelty, driving him to savage acts. He also carries a tell, tugging at the fabric of his clothes whenever he lies or hides something, a habit that recurs throughout the series.
A transgender man, Mutsuki carries deep scars from a boyhood under an abusive, alcoholic father. He joins the Quinx Squad as a surgically modified investigator serving beneath Haise Sasaki, whom he idolizes as a mentor and, in secret, holds intense romantic feelings for. His path darkens when the ghoul Torso, Karao Saeki, marks him and later abducts him, an ordeal that exposes his own ghoulhood and begins turning his hair white. In captivity he recovers memories of the abuse he suffered and the killings committed during his blackouts, among them a cat he had buried away as a child.

Consumed by jealousy over Touka, he stabs and assaults Uta while deluded into believing the man is Sasaki, an act his own protege Shinsanpei Aura helps keep hidden. He tracks Kaneki to Touka's coffee shop and attacks, his hatred flaring once he learns the two are married. Only after the other Quinx members reach him does his obsession loosen and his loathing for Touka fade. He mentors the Oggai Squads, briefly walks away from the CCG to chase Kaneki, then returns under the guidance of Urie and Saiko before transferring to a rural branch office alongside Aura and Ayumu Hogi. Years afterward he sends fruit to Touka, a quiet sign that the two have finally reconciled.
