Akaza is a heavily built young man of moderate height. His skin carries a pale green-white tint marked by thick blue lines that echo the criminal tattoos he wore in life. He has short pink hair that puffs around his head and yellow eyes set over a blue sclera, with the characters for his Upper Moon rank carved into each iris.

He goes mostly unclothed, wearing only a sleeveless purple-pink haori cut off at the waist and left open across the chest, plus baggy white trousers rolled below the knee and held by a turquoise cord. His relentless drive for power, it is later shown, stems from grief over loved ones he failed to protect as a human. By his final moments he recovers those buried memories, regrets his crimes, and chooses to end his own existence.

As Upper Moon Three, Akaza stands among the most formidable demons, surpassed only by the two ranks above him and Muzan. Even as a child without training he could knock out grown men, and at eighteen he overwhelmed dozens of armed swordsmen with his bare hands alone. After his transformation his strength climbed further, letting him battle the Flame Hashira and kill him, and later hold his own against both the Water Hashira and Tanjiro at once.

His Blood Demon Art, named Destructive Death, fuses the Soryu martial style he mastered as a human with controllable shockwaves that surge in blue and red. He launches these blasts from any limb at any range, and his Compass Needle technique senses an opponent's fighting spirit to predict strikes, locate vitals, and adapt mid-battle. His regeneration is extreme enough that, through sheer will, he briefly resists decapitation and even regrows his own head.
