Kagaya is a delicate man of medium height with very pale skin and black hair reaching his shoulders, parted at the center. A family curse makes him appear to decay while still living, a sickly pink withering that spreads from his brow over the rest of him, takes his sight, and blackens his eyes. By the close of his brief life it has covered everything, leaving him wrapped in bandages except for his mouth.
Even amid constant pain and frequent bleeding, he holds a calm, steady smile. He generally dresses in a black yukata under a flame-patterned white kimono, and Muzan would later call his presence physically sickening, reeking of a corpse.
Serene, devoted, and unshakable, Kagaya treats every member of the Corps as his own child, learning each by name and visiting the graves of the dead each day until sickness keeps him bedridden. Open about being unable to fight, he compensates through wisdom and compassion, standing up for Tanjiro and Nezuko when most of the Hashira opposed them.
