Biwako keeps her long brown hair gathered high in a ponytail, her dark eyes framed by faint creases at the corners that carry over to her mouth. She favours a purple kimono-style dress worn beneath a white haori, finished with sandals.

Flashbacks to her childhood show shoulder-length hair held by a pink flower band, a green kimono over a light-brown long-sleeved shirt, and a loquat stitched onto the back of her clothing.

Sharp-minded and something of a stern presence, Biwako matched her husband's wisdom and dispensed sage counsel on childbirth. She held firm views on the strength of women, insisting they endure what men never could, and she thought nothing of scolding Minato for letting his focus on the seal slip while Kushina laboured, Hokage or not. The anime adds her conviction that a mother, rather than the father, should be the first to greet a newborn.

In the anime, a young Biwako attended the Academy in the newly founded village, where Madara Uchiha's unsettling gaze frightened her until Hashirama Senju calmed her with a flower. Years later she and Hiruzen sat down with the expecting parents, Kushina Uzumaki and Minato Namikaze, not long ahead of the delivery.

Because the mother-to-be was a jinchuriki, the birth was moved to a guarded, secret site where Minato could hold the seal, with Biwako and Taji attending as midwives. She delivered the newborn Naruto safely, only to be slain by Obito Uchiha, whose attack had already claimed the Anbu standing guard outside.
