In the anime, a young Biwako attended the Academy in the newly founded village, where Madara Uchiha's unsettling gaze frightened her until Hashirama Senju calmed her with a flower. Years later she and Hiruzen sat down with the expecting parents, Kushina Uzumaki and Minato Namikaze, not long ahead of the delivery.

Because the mother-to-be was a jinchuriki, the birth was moved to a guarded, secret site where Minato could hold the seal, with Biwako and Taji attending as midwives. She delivered the newborn Naruto safely, only to be slain by Obito Uchiha, whose attack had already claimed the Anbu standing guard outside.
