Reflecting on how far Naruto has come, the Nine-Tails hears him swear that he will someday do something about the fox's own hatred, a claim the beast scornfully brushes aside. Out on the front, Neji pushes his Byakugan past its limit and collapses, so Kiba sends him back to the medical unit. Elsewhere, Nagato and Itachi discuss Akatsuki, their eye powers, and the intentions of the one who revived them, with Itachi noting that his Mangekyō Sharingan paired with Nagato's Rinnegan would be nearly unstoppable.

At the medical division, healers tend the injured until three jōnin-rank medical-nin turn up dead. With no enemy detected, suspicion lands on someone being controlled from within. Sakura insists the healing must go on regardless, while Neji sets out to expose the infiltrator. After Sakura turns down a shinobi's love letter, admitting her heart lies elsewhere, two medical-nin prepare to return to work, only for Neji to reveal himself as the murderer and strike them both down.

The chapter belongs to the Fourth Shinobi World War: Confrontation arc. One quirk in the artwork shows Itachi's eyes drawn without their usual black sclerae during his exchange with Nagato.
