Resembling Marshall D. Teach, Kurotsuru is a portly man with thin limbs, fluffy black hair tucked beneath a bandana on top, and a goatee. Round glasses sit on his face, and he wears a white undershirt bearing a cross, striped trousers, and white shoes, plus a physician's coat labeled Dr. Black Beard on the left with a medical cross at each collar and a pen in its right breast pocket. That strong resemblance is exactly why Ace took him for the genuine Blackbeard.
Kurotsuru holds a grudge against Ace for the kick to his face, angry enough to take part in tossing the pirate into a river.
Mistaking the doctor for the actual Blackbeard, Ace knocked him to the ground with a kick, enraging everyone nearby. After Ace recognized his error, the furious doctor teamed up with an irate cook Ace had skipped paying and a crowd of townspeople to boot the pirate into the river. Two years on, the World Government annihilated the Lulusia Kingdom, and whether Kurotsuru was present on the island at that time is not known.
