Rokai stands at ordinary height, with unkempt brown hair, a broad nose, and a long black beard. His pirate garb is a black coat whose shoulders are yellow and whose cuffs blend red and yellow, layered over a white button V-neck and a gold chain. A green bandana sits beneath a black and red hat marked with his old crew's emblem, while a patch covers his left eye. He looks almost exactly like Woonan, the beard being the single feature that tells them apart. When the curse rises, he dons a mask that gives his face the look of a bare skull, and in his ultimate Golden Skullface state his whole body turns gold while an Ancient Fragment hangs at his throat.

The savage, sadistic figure of Skullface conceals a man weighed down by grief. At heart Rokai wishes to keep others from chasing the Ancient Gold and falling under its curse the way he did, and his guilt over being made to slay his own crew runs so deep that he lets the curse rule him as a kind of penance. Even free of its grip, he urged the Set Sail Pirates to drop their hunt, warning that pressing on would strip away the very thing that defines a person as both a pirate and a man, then walking off rather than fighting. Beneath the mask, his stated aim shifts toward guarding the treasure itself instead of the people seeking it, a difference that renders Skullface far more brutal and far more willing to harm anyone in his way.

What Rokai can do in his ordinary state stays unknown, since he only ever enters combat as Skullface. In that guise he is a fearsome opponent, exchanging blows against the Straw Hats with a honed hook and able to wrap ships in a fog-cloaked calm so they can neither move nor spot their attacker. His most desperate transformation, Golden Skullface, repels strikes aimed at him and can pass the Ancient Curse to others. Fast and exact enough in that form to keep pace with Luffy's rubber gatling flurry, he turns each punch aside through moves like Golden Tiger Reverse and Golden Queen Reverse, while hook attacks such as Golden Hook Slash and his initiation techniques push the curse outward.
