Towering over the cities he crossed, this monster took the form of a colossal arthropod whose segmented body stretched roughly fifteen meters across at the start. Set into the middle of his skull sat a shriveled, elderly humanoid visage, while the insectoid face beneath it carried four luminous eyes, sweeping antennae, and a fanged maw. The manga rendered his plating in deep black, whereas the anime tinted the armor a dark violet. Compared to the smaller Junior Centipede and Senior Centipede, his design read as far more menacing. After shedding his shell partway through the clash with Bang, Bomb, and Genos, he swelled even further; his third layer of faces split open to expose a fourth, complete with an extra forehead eye, two more glowing eyes, and noticeably enlarged mandibles.

Where his lesser namesakes chattered and boasted, Elder Centipede stayed quiet and measured. He favored efficiency over spectacle, brushing past needless skirmishes to reach whatever he had been sent to seize, secure in the knowledge that his bulk and armor let him shrug off nearly anything thrown his way. He seemed to hold no personal agenda, content simply to serve as one of the association's senior figures. That calm hid a single obsession: a bitter grudge against Blast, who had nearly finished him two years earlier in a duel cut short by the intervention of "God." The wound left him warier and less smug than most monsters, and he leapt at any rumor of a rematch, even abandoning his brawl with Bang, Bomb, and Genos the instant King claimed the hero was near.

Ranked at Dragon level and counted among the mightiest monsters in the story, Elder Centipede paired catastrophic size with speed and raw force, prompting the Hero Association to liken him to a natural disaster. Gyoro Gyoro reckoned only four heroes could put him down, naming Tatsumaki, Metal Knight, King, and Blast. His strength let him flatten a city merely by passing through it, launch Metal Bat far across the skyline, and slice Genos apart with his jaws. Despite his scale he moved swiftly, burrowing underground at high speed and reacting fast enough to intercept a blast that had already been fired.

His defense proved even more formidable. A super-tough carapace turned aside Metal Knight's missiles and a full swing from Metal Bat, who had just felled two Demon-level foes in single hits. Beneath that shell, corrosive digestive fluids could eat through Genos's mechanical frame, and rapid regeneration let him regrow teeth, eyes, and incinerated organs within seconds. Each molt left him larger, tougher, and wearing an additional face, a process he used to recover after Bang and Bomb cracked his previous armor. He also weaponized his sheer mass through the Giant Centipede Tidal Wave, thrashing in a swirling surge that leveled everything nearby, and his frame doubled as a living transport for smaller monsters.

Only complete destruction could stop him, since anything short of that simply healed. That end arrived when King baited him into charging with a bluff about Blast, and Saitama, hidden behind the hero, unleashed a Serious Punch that vaporized the creature from head to tail and left only his severed claws. Because so few witnessed the truth, the kill was publicly credited to King.
