Ryouko cuts a slim, moderately tall figure with fair skin and brown hair reaching her chest, gathered loosely over her left shoulder and adorned with three small flowers. Her warm expression and caring eyes are the very features her daughter Hinami would inherit. For clothing she pairs a soft pastel dress cinched by a braided orange belt with a pale yellow sweater and a reddish scarf. Kaneki thinks of her simply as a pretty mother.

Tender by nature, Ryouko shies away from hunting or killing even though her ghoul biology demands flesh. Her devotion to Asaki runs deep, as she stands by his questionable history and insists that he is a decent man, leaning on him to keep the family safe. Losing him leaves her grieving and heavy with guilt, and she eventually apologizes to Hinami for the burden she placed on her husband and for the peril her daughter now faces. Resolving to build a new life, she prepares to leave Anteiku, and in her final moment her only wish is for Hinami to survive.

Ryouko first appears alongside Hinami at the family clinic, where the two duck behind a curtain as Yakumo Oomori storms in to interrogate Asaki. Sensing the mounting danger, she turns to Yoshimura at Anteiku for help securing prepared meat, since neither she nor her daughter can hunt on their own. Word of Asaki's death soon reaches them, and mother and child mourn together while investigators comb the 20th Ward. After visiting her husband's grave to bury his mask, she is escorted back by Renji Yomo and tells Yoshimura that she intends to move on and start over.

She and Hinami set out through a rain-soaked Tokyo, only to walk straight into Kureo Mado and Koutarou Amon. Ryouko unfurls her koukaku kagune, which opens like spreading flower petals capable of both shielding and striking, and orders Hinami to flee, hoping to trade her own life for her daughter's. Judging her too harmless to fight or hunt, Mado carries on with the execution and slices through her neck before she can finish her parting words. Her kakuhou is later harvested and combined with Asaki's kagune to forge Mado's newest quinque, Fueguchi Two, and in an afterlife she is reunited with her husband.
