As the fit members and the Public Safety defectors work to fit Denji back together, Nobana turns up the heart. Asked why she will not help, Yoru points to her wound while Katana Man brushes the job off as something for children. Haruka rejoices over holding his hero's innards, Seigi wonders about Chainsaw Man's genitals, and Katana Man answers with a vulgar quip that Yoru rejects, since she needs Denji whole for the coming battle. The Nail Fiend needles her over being too young to grasp romance, leaving Yoru red-faced and shouting. When Nobana finally pulls out the head, Haruka hurries over, and the buried truth comes out: Denji and Chainsaw Man are one and the same. Asa is stunned, but the reattachment halts as the opposite door caves in. Takagi, the bodyguard placed over Denji, charges in and tells his support, Quanxi, to slaughter every intruder in sight.
Asa's crew overpowers the surgeons and discovers Denji packed into boxes in pieces. At Yoru's command the group rebuilds him, with Nobana recovering first the heart and then the head.
The retrieved head reveals to Haruka and Asa that Denji has been Chainsaw Man all along. The work stops abruptly when Takagi crashes through a door and sets Quanxi loose on them.
