At first Sloth is little more than a twisted heap of bone and flesh left behind by the brothers' botched transmutation, but feeding on red stones from Dante reshapes her into a normal woman who mirrors the late Trisha Elric. She keeps her hair loose rather than in Trisha's side ponytail, and her disguise copies Trisha's coloring exactly. Once she drops the Juliet Douglas cover, her hair darkens to black with greenish-brown streaks and her eyes shift to a pale violet. Two looks define her: a lavender office ensemble with a green collared shirt and short skirt for her secretary role, and a fitted dark purple gown with long gloves as her true self. Her Ouroboros mark sits over her left breast, where her heart would be.
Named for sloth yet far from lazy, this Homunculus reads the sin instead as apathy, coming across as the coldest and calmest of her kind. She once drowned a man without so much as looking up from the cup in her other hand. A darker shade of the sin, sorrow, also fits her, since her borrowed memories of Trisha's family life torment her and drive her wish to be rid of them. Though the youngest Homunculus, she ranks near the top before Wrath arrives, even giving orders to Envy, the eldest, a standing likely owed to her unshakable composure against his hot temper.

In her last stand at an abandoned warehouse she nearly wins Alphonse to her side, but a bone from the real Trisha weakens her and Lust turns against her. After Wrath absorbs Trisha's remains and embraces her, she is left immobile, and Edward transmutes her body into ethanol so that she evaporates while offering her sons a final piece of motherly advice. She is the third Homunculus to die.
