A broad birthmark spreads across his slightly bulbous nose, a feature paired with dark hair and equally dark eyes. In childhood he wore a cap fixed with three pins bearing the orphanage's crest, along with a black shirt, white shorts, and sandals. Grown up, he dresses in the ordinary gear of a Konoha ninja, flak jacket included, and once the war began he transferred those same orphanage pins onto the Allied Shinobi Forces headband he now wears. A short goatee rounds out his adult look.

Urushi speaks his mind with little restraint and can come across as rough, as when he barked at Kabuto to thank the nun who healed him or smacked a helmet onto the boy's head so he would stop hurting it. Underneath the bluntness sits real warmth. He took in the nameless stranger without a second thought, treated the orphanage's other children as his own brothers and sisters, and, during the Fourth Shinobi World War, instantly offered a bed at the orphanage to a comrade with nowhere to go. His feelings toward Kabuto ran deepest of all: he counted him as a brother and spent years certain he would come back, never once entertaining the thought that Kabuto might have died.

As a child living at the orphanage with Nonō Yakushi and the other kids, Urushi came across a badly hurt boy lying beneath a tree. Once the boy had been tended to, Urushi helped walk him back to the orphanage. When it emerged the next day that the stranger had no name, the nun settled on Kabuto, taken from the helmet Urushi had planted on the boy's head. In the years that followed, the orphans assisted Nonō and Kabuto in treating Konoha's ninja; Urushi grumbled about aiding fighters but recognised the work paid the bills and did his part. His own skills lean toward first aid rather than combat, which he put to use on the battlefield of the Fourth Shinobi World War. Patching up a fellow soldier late one night, he wished aloud for the fighting to end so he could finally sleep, and spoke of waiting for his brother Kabuto to come home. Years later, when Naruto brought Shin Uchiha's clones to Konoha, Urushi looked on quietly as Kabuto introduced himself to the group.
