To all appearances she is a girl of about ten, yet her real age is twenty-two, her body locked in childhood by the fruit she consumed. Aquamarine hair falls to her shoulders above dark blue eyes. As the Donquixote crew overran the kingdom, she appeared in a yellow dress paired with a hood shaped into bear ears. Down in the underground trade port her outfit changed to a white dress dotted with pale blue, a magenta bear-eared cape, a small crown, and open-toed sandals, finished with a pink-framed monocle and purple lens set over her left eye in the manner of an eyepatch.

Sugar serves Doflamingo with unwavering loyalty and helped him seize the kingdom, showing genuine cruelty as she damned citizen after citizen to a forgotten life of toy slavery and took pride in doing so. A trace of empathy still flickered in her, seen in the sadness she felt for dwarves who kept faith in a hero who seemed to have left them behind. Much of the time she stayed indifferent to events nearby, though she could act every bit the child she resembled, racing through the palace hunting for a Doflamingo who had already departed. Beneath the harmless mask she was sly, able to cry on command and dangle grapes as a lure. Her undoing was a fragile nerve, for Usopp's tortured face once frightened her into a faint and left her dreading anything that resembled a long nose.

Doflamingo pulled Sugar and her older sister Monet out of a miserable existence, Sugar at nine and Monet at seventeen, and the pair devoted themselves to the Donquixote Family. At age ten she consumed the Hobi Hobi no Mi, whose Paramecia power turns anyone she touches into an obedient living toy while scrubbing that person from the memory of all who knew them, and the same power froze her aging. Every victim is then bound by a contract whose rules she sets, and should she ever black out, all toys revert and the lost memories rush back. Her first known transformation, Kyros, occurred while she shielded Doflamingo, though a missed contract let Kyros keep his will. Across ten years her toys grew to include soldiers, pirates, Marines, royalty, and even animals.

Throughout the Dressrosa Arc she kept converting colosseum losers and prisoners inside her executive tower while Trebol stood guard, mistakenly promising her that nobody knew of her. Usopp, Robin, Kyros, and a dwarf army were in fact closing in. After Robin drew Trebol off and the dwarves rushed her, Sugar turned several of them and snared Robin as well. The scheme to knock her out with a Tatababasco-laced grape failed at first, so she crammed the spiced fruit into a battered Usopp's mouth, only for his agonized expression to send her crashing into unconsciousness. That collapse released every toy in the kingdom at once and began unwinding Doflamingo's grip.

Revived afterward by Mansherry's healing, she woke with a dread of long noses, assembled an army of giant nutcrackers, and swore to take revenge on Usopp. As she closed in to erase Luffy and Law, Usopp sniped her from a distance with a recreated image of his terrifying face, dropping her again and saving the two. She was carried to the timeout room with the other beaten officers and, following Doflamingo's defeat, arrested by the Marines.
