Summary

Feeling J. Geil's death through the bond of shared blood, Enya breaks down in grief and readies her revenge, dispatching a subordinate armed with the Empress Stand. Meanwhile, Jotaro's party rides a bus toward Varanasi, a city steeped in religious tradition, and is joined by the woman calling herself Nena. Once there, Joseph frets over what he takes for an insect bite on his arm and heads to a hospital, while Nena, a strange wart on her tongue, remains with Polnareff.

When the doctor tries to cut the growth away, it kills him, sprouts a face of its own, and reveals itself as Empress. Joseph struggles to fight the Stand fixed to his body, only for it to pin the killing on him and set the police on his trail. The Ripple proves useless because the Stand is part of his own flesh, so Joseph instead uses Hermit Purple to squeeze it and wrench himself free.

Hunting for the Stand's true master, Joseph scatters chaos through the streets while dodging Empress as it grows steadily larger and stronger. By smashing pots and casting Hermit Purple over the shards, he assembles a map that points him toward something useful. He tracks down a barrel of coal tar and plunges Empress into it. The Stand scoffs that it cannot suffocate since it is not alive, yet the tar hardens around it and freezes it in place.

Joseph explains that the map and some ashes led him to the tar, then tears the immobilized Empress to pieces with Hermit Purple. At that same moment Nena is ripped apart across town, exposed as the user, her true shape an obese woman who had worn a younger woman's skin as a disguise. Reunited with Jotaro and Kakyoin, the group sets off by car for Pakistan, unaware that a second vehicle has begun to follow them.

The adaptation stages Enya learning of her son's death in the courtyard of DIO's mansion rather than its foyer, and adds scenes of her striking a stray cat and suffering psychic wounds like the ones that killed J. Geil. It skips the religious rites of Varanasi, softens the doctor's brutal killing, and closes with an extra beat in which the group prepares to drive off and Wheel of Fortune begins its pursuit.

A small nod appears in the background, as the symbol of Morioh can be seen at the clinic's entrance. Commentary praised the melancholy atmosphere of the holy city and the dogged back-and-forth between Joseph and Empress, noting how diminished Joseph appears compared to his prime before his old cleverness resurfaces.
