With Mahito having transfigured her into a near-hopeless condition, Arata treated her and, despite his own private doubts about her chances, would not extinguish Yuji's hope outright. Having just seen Yuji collapse in tears, he picked his words to leave a thread of optimism without overselling it, stressing that her slim odds were nonetheless not zero.
Throughout the Shibuya Incident, Arata moved through the turmoil at Aoi Todo's side, the pair coming upon throngs of unmoving people. Once Todo grasped that the Prison Realm had been carried off, the two set out after both it and Yuji. Arata showed up after Todo had already seen to Nobara and was asked to heal Yuji, learning along the way that Sukuna had killed countless people, Nanami included among the dead. As Todo took on Mahito, Arata used his technique on Yuji, stopping the bleeding and freezing his current wounds from worsening, then announced he would pull back with Nobara, her survival still in doubt.
By the time of the Shinjuku Showdown he sat in with the sorcerers mapping their offensive against Sukuna before the Gojo fight, going over his technique in a session with Yuji, Shoko, and Rin Amai. On December 24, partnered with Rin and Shoko, he applied his ability to stop Yuta Okkotsu's terrible wounds from declining further, realizing to his horror that the gash Sukuna had left matched Gojo's and sat past anything Reverse Cursed Technique might repair.
