Devouring another finger to deepen his hold, Sukuna decides to kill Megumi for no real reason at all.
Believing Sukuna's reverse cursed technique gives him an opening, Megumi chooses to fight, hoping to force a heal. He summons Nue and Great Serpent, but Sukuna shreds the shikigami and overpowers him with raw agility alone. Reflecting on the unfairness of the world and his own conviction to save people unequally, Megumi declares he never regretted sparing Yuji. Stirred by the words, Yuji briefly reclaims control before succumbing to his wounds, telling his friends to live long lives.
In Tokyo, four disguised figures move through a crowd discussing why a Sukuna finger was used to gauge the curse's strength, eventually settling at a cafe where only the human among them is visible. Gojo meets Ijichi in the school morgue, convinced the higher-ups deliberately sent Sukuna's vessel into danger during his absence, and threatens them as Shoko Ieiri requests to dissect the body. Grieving the loss, Megumi and Nobara are introduced to the second-years, Maki, Toge, and Panda, and agree to join the Kyoto Goodwill Event to grow stronger. The cursed spirits, meanwhile, plot war against humanity, naming Gojo's neutralization and Sukuna's alliance as their two conditions.
The episode covers chapter 8's final pages through chapter 10 and introduces a large cast including Kenjaku, Jogo, Hanami, Dagon, Shoko Ieiri, Maki, Toge, and Panda. The heart-removal scene is longer and more graphic than its manga counterpart, and Megumi's monologue on life's unfairness gains added imagery of Tsumiki.
