Adapted from chapters 24 to 55 of the Boruto: Naruto Next Generations manga, this arc is split in the anime into two runs, Kara Clash across episodes 188 to 205 and Ōtsutsuki Awakening across 206 to 220. Both versions follow the Ao Arc; the manga leads into the Code's Assault Arc while the anime continues with the Chūnin Re-Examination Arc. At its heart lies Kawaki, a boy engineered by Kara as a vessel, and the escalating clash between Konoha and the organisation that made him.

Summary

Recovered by Team 7, the volatile Kawaki is brought to Konoha, where Naruto insists on giving him a taste of ordinary life despite his hostility. A broken vase, clumsy first steps into everyday joys, and flashes of trauma slowly wear down his guard, and Naruto's patience begins to reach him. Kawaki reveals that Jigen forced the Kāma seal upon him after countless other children died in the attempts, and a bond starts to form between the boy and the family. When the Kara Inner Delta storms the village to reclaim him, Naruto overpowers her, but she escapes, having learned that Boruto also bears a Kāma.

Jigen soon comes himself. He effortlessly subdues Naruto, and to spare the village Kawaki agrees to surrender, but Naruto refuses to cooperate, so Jigen banishes him to another dimension. There Naruto and Sasuke uncover Jigen's true nature as they fight: he can shrink matter and himself to a microscopic scale, and he draws power from a hidden Ten-Tails to assume a monstrous transformed state. Outmatched, Naruto is sealed inside a coffin while Sasuke barely escapes, and Jigen collapses, revealing that an entity named Isshiki Ōtsutsuki has been possessing his failing body all along.

A reformed Team 7 led by Sarada crosses into the other dimension to rescue Naruto and confronts the regenerating Boro. They eventually destroy his healing core, but a rampaging Boro is finished only when Momoshiki briefly seizes control of Boruto. Back at Kara's base, the scientist Amado defects to Konoha and exposes everything: Kara means to grow a God Tree, Jigen is a human turned Ōtsutsuki, and the Kāma will one day overwrite its host, dooming Boruto to become Momoshiki. Amado's ally Koji, revealed as a Jiraiya clone, burns Jigen to death, forcing Isshiki to be reborn fully in that body.

Isshiki tears through Koji and invades Konoha to reclaim Kawaki, dragging Naruto, Sasuke, and Boruto into a final dimensional battle. With his allies outclassed, Naruto accepts Kurama's offer of Baryon Mode, a transformation that drains the enemy's lifespan at the cost of the user's own, and slashes Isshiki's remaining hours down to minutes. Kawaki then tricks Isshiki into branding a shadow clone, denying him a new vessel, and the Ōtsutsuki finally perishes. The victory is immediately soured as a Momoshiki-controlled Boruto destroys Sasuke's Rinnegan before being subdued, and Naruto learns that Baryon Mode spent Kurama's life, not his own. As the fox fades, Naruto loses his partner's chakra forever, while far away, Code inherits Isshiki's will and vows revenge, promising to feed Boruto or Kawaki to the Ten-Tails and become a god himself.
