In her younger days Koharu was a striking kunoichi with dark eyes, her hair drawn up into twin buns held by a traditional pin from which two pearls hung to one side, while fringes hid her forehead protector. Her combat outfit paired a plain black garment with a grey breastplate and arm guards, fishnet tights beneath a skirt, a bandage-filled pouch fixed at her back, and standard sandals. Reasoning that the enemy knew neither their exact position nor their strength, Koharu urged patience and a counter-ambush, and she then watched as Tobirama gave himself up as bait so the rest could slip away. When Hiruzen rose to become the Third Hokage, she and Homura took their places as his advisors, and through experience and tireless work they earned standing nearly equal to the Hokage himself.

Koharu enters the series proper at a Council session with the village's jōnin, where she cautions against fixating on Orochimaru as the sole threat, noting that Anbu are already gathering intelligence abroad. Following the Third's death, she and Homura pressed Jiraiya about selecting a new Hokage, and when he declined the seat in favor of another candidate, she acknowledged that Tsunade qualified despite her unknown whereabouts. Later dealings kept her in the role of a wary counselor: she and Homura repeatedly raised concerns over how freely Tsunade let Naruto operate, and during Pain's invasion the pair urged that the boy be kept hidden away.
