Baby 5 is a tall, slender young woman who wears lipstick and a burgundy maid outfit trimmed with white frills, matching heels, and an array of tan belts for holstering weapons. Long black hair, lightly curled and topped with a white lace band, frames her face, and she is often seen with a cigarette. Heavy arms ride on her back at times, from a large rifle and an axe to a sword. Arriving at Punk Hazard, she added goggles that fill with tears when she cries, along with a bazooka and a Gatling gun. In flashbacks she appears far younger: shorter hair and a yellow ribbon at age eight, then less wavy locks, a red ribbon, and plain brown gloves at fourteen.

Abandoned in the mountains by a mother who branded her useless, Baby 5 grew consumed by a craving to prove herself needed by anyone at all. She physically cannot refuse a favor and lights up the instant someone asks something of her, even when the request was plainly a joke, going so far as to attempt suicide to satisfy it. The compulsion leaves her willing to marry, fund, or buy from anybody, saddling her with a debt of 98,000,000 Berries and eight late fiancés. Yet she also bristles at being called convenient, weeping when Lao G used the word. Beneath that lies a deeply emotional streak: she repeatedly tried to kill her own captain for murdering her suitors, and she cries and runs to Buffalo for comfort whenever Law glares at her, just as she did in childhood when she and Buffalo played pranks on the clumsy Corazon.

Born in Kuen Village to a family that treated her as a burden, she was cast off in the mountains and later recruited into the Donquixote Pirates, where she ate the Buki Buki no Mi. That Paramecia fruit lets her turn all or part of her body into any weapon she imagines, from swords and sickles to pistols, cannons, and even a missile; if a weapon's effect blasts her apart, she reforms, though plain external damage does not heal that way. As a child she and Buffalo endured a training regimen that scared off a hundred other recruits, and she befriended the new arrival Trafalgar Law, growing sympathetic once she learned of his doomed hometown of Flevance. Over her years in the crew she rose to officer in Pica's army, holding sway over two thousand lower members, and Doflamingo killed all eight of her fiancés and their towns to shield her from their motives.

During the Punk Hazard Arc she lashed out at Doflamingo for slaying her latest love, then was dispatched with Buffalo to retrieve Caesar Clown. The pair clashed with Franky's General Franky, were struck down by Nami and Usopp, and were briefly decapitated by Law before reattaching. In the Dressrosa Arc she stood among Doflamingo's officers through the Birdcage crisis and shot a grunt for mocking Pica's voice. Her fight with Sai turned her life around: she mistook his remarks for romance, nearly killed herself when he sarcastically told her to die, and was overjoyed when, after beating Lao G, he vowed to make her his wife. Defecting from the Donquixote Pirates, she was saved by Sai when Pica tried to crush her and watched Zoro topple Pica. She helped the Straw Hats flee Dressrosa and joined the new Grand Fleet, and after Sai broke off his prior engagement to Uholisia, the two were wed, fulfilling her dream of becoming a wife.
