The 784th One Piece episode sees Luffy poisoned by a deadly fish just as the Sanji Retrieval Team crosses into Big Mom's domain and runs into a Germa 66 vessel carrying two of Sanji's siblings.
After days of hunger, Luffy finally hooks something big while fishing, but the giant fish slips back into the sea and fights his attempts to land it. With a colossal group effort the team hauls it onto the Thousand Sunny to cook. Chopper, unsettled by its odd coloring, digs through Sanji's locker and uncovers a fish reference book buried beneath magazines, learning that the creature's skin is lethally poisonous. He rushes to warn the others, only to find Luffy already chewing on the skin. Luffy feels fine, and Chopper explains the flesh is safe if skinned and prepared per Sanji's notes.
Nami and Carrot cook the fish to the recipe and serve generous portions, which the team finds delicious. But Luffy soon collapses, his body shutting down from the skin's toxin. Chopper, lacking proper supplies, panics and weeps; though Luffy clings to life despite a poison that normally kills instantly, they urgently need herbs from an island.
Pekoms cautions that stopping is impossible since they are entering Big Mom's Totto Land, confirmed when the Den Den Mushi rings with a Territorial Sea Slug warning. Cotton-candy snow delights the crew before a mysterious ship looms close. Expecting a Tarte scout, Pekoms instead finds it belongs to Germa 66. A hooded man whose face mirrors Sanji's approaches, and the overjoyed team begs him for an antidote, mistaking him for their cook. He coldly reveals he is not Sanji but his younger brother Yonji, refusing to explain their bond or to help. When Yonji catches sight of Nami and goes lovestruck, the family resemblance becomes obvious. As tensions flare, his sister kicks him off his own ship for being stingy and leaps onto the Sunny, simply apologizing for his behavior.
The anime adds Luffy insisting he was fine right after Chopper's warning, shows Nami cooking the fish, and has the team briefly hide upon spotting the Germa ship. In the manga Luffy pulled the giant fish out only once, whereas here it leaps back in and nearly drags him overboard before Chopper, Brook, and Pedro help reel it in. Unlike the manga, Luffy does not foam at the mouth while poisoned. This episode continues the Whole Cake Island Arc.
