Kouto was a boy ghoul from the 11th ward who belonged to Kazuichi Banjou's circle during Banjou's time governing the district. The son of a woman named Kei, he met a brutal end at the hands of Yamori after a botched breakout from Aogiri's base.
Kouto dressed in the ordinary garb worn by those attached to the Aogiri Tree, giving little to distinguish him from the wider organization around him.
Soft-spoken and reserved by nature, the boy still held real warmth for the people close to him. His courage came through in small, desperate acts, from swiping food when hunger gnawed at him to placing himself in harm's way to shield his mother.
Kouto took part in the Anti-Aogiri gathering of ghouls loyal to Banjou, where the group plotted a way out of the 11th Ward Base. Later, made to help dismember human corpses on Aogiri's orders, hunger drove him to steal a piece of meat, and Banjou absorbed the punishment on his behalf.
Once the escape attempt collapsed, the boy and his mother were dragged before Kaneki. As the two trembled together, Yamori demanded that Kaneki spare only one of them and condemn the other. Kaneki failed to choose, and Yamori strangled Kouto to death in front of his mother, then killed her moments afterward. In the anime, he and Kei were swapped out for the characters Shuu and Haru.
