Because Choso is born from a Death Painting Womb, his flesh-and-blood frame stays visible to ordinary people rather than hiding like a normal curse. His most recognizable trait is the crimson streak that crosses the bridge of his nose and runs out toward both cheeks, a mark he can deliberately make bleed to supply ammunition for his technique. The pattern reshapes when he activates Flowing Red Scale. Above that, dark stringy strands rise in two tall ponytails that point upward and out, framing narrow eyes and thin brows.

For attire he layers a slack tan robe beneath a purple vest cut into a V across the chest, the lower portion hanging to the waist, finished with a matching round neck scarf and brown boots. The anime version tints his hair lighter and adds violet shadow around the eyes.

Quiet and detached, Choso usually wears a bored look and drifts to the edge of any group, rarely speaking unless the topic touches his family. Although he carries cursed-spirit blood, he holds no built-in urge to harm people and feels no contempt for sorcerers. He loved his human mother, and the only being he truly hated was the sorcerer who toyed with his brothers. Beneath that flat exterior runs fierce devotion to his siblings, whom he sees as a single shared existence.

News of Eso and Kechizu dying shattered his calm and pushed him to seek vengeance during the Shibuya Incident, even as he stayed deliberately out of Gojo's reach and clashed with Jogo over whether Yuji should be spared. When he sensed Yuji's impending death the same way he felt his brothers' ends, he stopped short of killing him. Realizing Kamo's body-hopping made Yuji kin, he claimed the boy as another younger brother and redirected his fury at Kenjaku. Pride in being the oldest of ten anchors his whole identity; an elder, he insists, must walk ahead and either model the road or warn against it, which is why he cannot fathom someone like Naoya who despises weaker brothers. He blames himself for choosing to live as a curse and dooming a future where all the siblings might have fought together, a regret he confessed to Yuki before honoring her by carrying her notes to Yuji.

Tengen later chose him among three guardians against Kenjaku, where he fought strategically beside Yuki and Tengen, drew out his father's hidden technique with inventive blood work, and ambushed Kenjaku after feigning his exit. He eventually learned reverse cursed technique through replacement training before the raid on Sukuna and even coached Yuji on healing fatal wounds.
