Fearsome Womb, Part 5 turns Sukuna loose in Megumi's body against his former comrades. Channeling the Angel's power, Hana strikes back, only to be deceived by the curse's true nature.
Now controlling Megumi Fushiguro, Sukuna unleashes terror on the people who once fought beside Megumi. Refusing to abandon him, Hana Kurusu draws on the Angel's strength to challenge the Disgraced One head-on.
A flashback clarifies Sukuna's intent. During their earlier fight, he sensed both the potential of the Ten Shadows Technique and Megumi's capacity to suppress him, so his goal all along was to claim Megumi's body and the technique with it. He could have forced a finger on him sooner, but feared Megumi might prove a mere cage like Yuji rather than a true vessel, so he waited until the moment Megumi's soul was crushed to guarantee control.
In the present, Yuji bleeds from his missing finger and cries out for Megumi, but it is Sukuna who answers, having suppressed him. Sukuna blitzes Yuji at terrifying speed and smashes him through an entire building. As Maki and Takaba rush toward the disturbance, Sukuna summons Nue, his version larger and more imposing than Megumi's, and rains lightning down on them. He is then struck from behind by Hana's beams of light, which blind and burn him; Sukuna realizes the Angel revived her quickly. Though Hana does not want to harm Megumi, the Angel insists they must risk it before Sukuna strengthens his grip. Channeling the Angel, Hana fires the extension technique Jacob's Ladder, a towering cylinder of light, while begging Sukuna to give Megumi back. Sukuna survives but appears injured, then feigns being Megumi regaining control, claiming to remember Hana and beckoning her into an embrace. Despite the Angel's repeated warnings, Hana clings to him, and at that moment Sukuna's head warps into a monstrous form poised to tear her apart.
Part of the Culling Game arc and collected in Volume 24, the chapter explains Sukuna's long-term plan to obtain the Ten Shadows Technique and showcases his enhanced Nue. It begins the Yuji Itadori, Maki Zenin, and Hana Kurusu versus Sukuna battle, with Hana wielding the Angel's Technique Extinguishment through Jacob's Ladder before falling for Sukuna's deception.
