As Ivankov entertains his followers, Bellett interrupts to rage that his father, once a king, returned from Kamabakka Kingdom transformed into an okama, ruining the family and reducing the prince to a pirate. He fires his cannon in revenge, only for Ivankov's Death Wink to bounce the shot back before female hormones reshape him into a curvy woman who flees in shame. The episode covers pages ten to nineteen of Chapter 537 and pages two to seven of Chapter 538 within the Impel Down arc. Bellett debuts, the Horu Horu no Mi is introduced, and Ivankov unveils Newkama Kenpo, Death Wink, Emporio Onna Hormone, and Emporio Chiyu Hormone. While running on stilts, Buggy and Galdino break the fourth wall to explain bamboo stilts to younger viewers. This marks the first flashback of Ace and Luffy as children, though Ace's shirtless adult look and beaded necklace conflict slightly with the manga timeline.
