Chapter 610: Madam Shyarly, Fortune-Teller

In Coral Hill, Sanji wakes after a transfusion and learns his donors were a pair of okama, the twins Splash and Splatter, which leaves him foaming in shock. Chopper, examining Luffy, finds a puncture on his arm and realizes he was poisoned, yet shows no symptoms thanks to antibodies he built up after surviving Magellan's poisons in Impel Down. Luffy credits the poison to a sword-wielding octopus merman he encountered, later identified as Hyouzou, who serves the New Fish-Man Pirates as a killer. Meanwhile, the barrel that had imprisoned Caribou is found broken open and empty.

At the Mermaid Cafe, Camie speaks with her boss, the fortune-teller Madam Shyarly, asking her to stay away from Sanji since her beauty would endanger him, and to keep his whereabouts secret. Camie tells the crew that Shyarly is a renowned seer who correctly called the Great Age of Pirates, the Marineford war, and Whitebeard's death, and has come to despise her own gift. Pappag, it turns out, is in the cafe fussing over an old friend who proves to be Brook. The reunion brings out a flood of Brook's skeleton jokes.

Pappag explains the threat hanging over the island: Vander Decken IX has been sending the Mermaid Princess escalating, threatening marriage proposals, prompting King Neptune to form an army under his three sons. The Vander Decken they met is the cursed captain's descendant, the ninth, a wanted criminal. Pappag also reveals that since Whitebeard's death, the Emperor Big Mom, real name Charlotte Linlin, now protects Fish-Man Island in exchange for monthly candy. As the tour reaches Gyoverly Hills, Shyarly causes a panic back in Coral Hill, declaring that her vision shows the island destroyed by the straw-hatted Luffy's hand.

Sanji survives thanks to the okama Splash and Splatter. Luffy is shown to have gained poison immunity after Magellan poisoned him in Impel Down. Madam Shyarly, who runs the Mermaid Cafe, makes her first appearance, and the blue-ringed octopus merman Hyouzou receives his name. Caribou breaks out of the barrel and abducts the mermaid Ishilly. At the cafe, Brook and Pappag are found again by Luffy and Usopp. Vander Decken IX is identified as a wanted descendant of the legendary captain. Big Mom is revealed as the island's protector, and Shyarly foresees Luffy destroying Fish-Man Island.

This issue gives the first reference to King Neptune, father to both the Mermaid Princess and the three princes, with his daughter established as the Neptune Family's youngest member. As payment for guarding the island, Big Mom demands candy, which Pappag suspects is a purely business arrangement rather than the kind of unconditional guardianship Whitebeard offered. Shyarly stresses that the vision came against her will; sensing something off about Luffy pushed her into glimpsing what was coming, and she prays she is mistaken.
