As Komachiyo carries Luffy, Zoro, and the feverish Tama, Hawkins sends his straw monster after them. Zoro fends off the creature and cuts it down, and once the group breaks free, a woman concealed inside the komainu's tail emerges and guides them to her teahouse to treat Tama.
Hawkins draws the Reverse Hierophant and looses his straw monster, armed with a spiked bat, to chase down Komachiyo. Zoro turns to face it and lands blows, but Hawkins shunts the damage onto his own subordinates. When the creature rains nails and then bears down on Komachiyo, Zoro severs its hand and finishes it with Nigiri: Toro Samon, slicing it apart so that it crumbles. Hawkins, unharmed thanks to his redirection, draws a third card, the Hierophant, and foresees that someone will steer Luffy and Zoro toward safety.
The group escapes Hawkins, but Tama's fever climbs as Komachiyo reaches a fork in the road. A woman who had stayed concealed within the tail this entire time steps out and names herself as the person Zoro saved earlier from pirate attackers. She directs Komachiyo to Okobore Town and her teahouse, where a remedy for Tama can be brewed. There, a waitress is harassed by the sumo wrestler Urashima until the arriving Zoro frightens him off. Luffy grinds herbs while Tsuru prepares the tea, all unaware that Bepo and other Heart Pirates are observing from a distance before reporting to Law.

The closing beat shows Kaidou resting on Onigashima with fury in his eyes. Anime-original additions include Zoro complaining about how irritating Hawkins's power is, Komachiyo getting lost before Tsuru appears, Kiku serving dango before Urashima scares the customer off, Zoro's intimidation of Urashima being shown rather than only mentioned, and Luffy assisting with Tama's care. Bepo grows depressed when Shachi notes he stands out as a bear, and Law is shown pondering in an abandoned home. Zoro's fight against the straw figure is extended: he removes his haori, uses more than one strike, Komachiyo is briefly knocked out, and the figure vanishes after being cut in two.
