Poised over the self-destruct switch, Monet suddenly feels her own heart pierced and crumples, blood spilling from her mouth, just as Caesar gleefully drives a blade into the heart he believes belongs to Smoker. Smoker stands unharmed, and the dying Monet reveals a cube-shaped hole in her chest. Doflamingo, listening over the Den Den Mushi, senses something has gone wrong as her line falls silent.

Aboard the cart, Smoker presses Law on how the surgeon held his heart the whole time. In flashback, Law recalls that after his clash with Smoker he held two hearts, and rather than keep Smoker's, he quietly returned Monet's to Caesar without the scientist noticing the switch. Trusting that good deeds bring good fortune, Law gave the secretary's heart back, an act that just saved the Marine's life.

At the tanker, Baby 5's missile fails to finish General Franky, and their duel resumes as she reshapes into a revolver only for Franky to shrug off the blast. Inside the crumbling SAD corridor, the explosion from the production room sends rocks raining down, and Law warns that a cave-in would bury them all. Zoro casually slices through the rubble blocking the path. Law then asks whether anyone can generate wind for their gas-choked exit, and Nami surprises the doubtful Marines by volunteering.

Unable to reach Monet and confirming that Punk Hazard was never destroyed, Doflamingo is furious. He dons his feathered coat and leaps from a high window. On Dressrosa, a young girl notes he has slipped away alone again, while an old man responds with eerie calm. Racing across the sky over the sea, Doflamingo brushes off Baby 5 and Buffalo's struggle and declares he is coming to the island himself.

Adapted from Chapter 694, the episode rates an 8.8. The hole in Monet's chest is hidden from view here, likely a censorship choice given the manga showed it across her exposed cleavage. The anime also reorders events, having Caesar stab the heart in sync with the SAD blast rather than after it, and shows Law returning Smoker's heart only once the rail car is found. Added touches include the cart cheering Zoro after he clears the debris and the gas pursuing them down the tunnel.
