Koto Shinazugawa was the youngest of the Shinazugawa siblings, the little brother of Sanemi and Genya. Still only a small child, he died alongside most of his family the night their mother, turned into a demon, attacked their home.
Koto was a young boy with light skin and large, round blue eyes. His dark hair was cropped close all over his head.
He bore a clear resemblance to his mother and to his brother Genya, and he dressed in a simple dark brown kimono.
As the baby of the family, Koto showed the natural traits of a very small child, curious about the world yet trusting of those who cared for him.
The depth of that trust is captured the night his siblings waited up for their mother, when Koto slept peacefully in Genya's arms, secure and undisturbed in his brother's hold.

Koto was the youngest of seven children born to Shizu and the violent Kyogo Shinazugawa, with Sanemi and Genya the eldest. His father frequently abused the family, though Koto was always shielded by his mother, and Kyogo was eventually killed in a stabbing.
One night, with their mother still not home, Sanemi went out to search while Genya stayed to watch the younger children. A knock came at the door, which then burst open to reveal Shizu transformed into a demon. She killed four of the siblings at once and slit Koto's throat as she lunged past, and despite Genya's efforts the child bled out and died. Long after, in the afterlife following the final battle with Muzan, Sanemi briefly glimpsed Koto reunited with their fallen family.
