A black fur coat, a battered wide hat, and a noose-like cord at his neck give Doc Q the look of a Western undertaker, an impression deepened by his scythe, gaunt build, and dark dress. Beneath the coat sit a green turtleneck and a brown robe, his hairy legs showing, and his feet end in black, heavily buckled shoes. He is huge, as his crewmates all are, and seldom dismounts from his oversized horse Stronger. Straight purple tattoos run from his hairline toward the corners of his eyes, his nose stretches a bit longer than usual, stubble shadows his jaw, and long ash-blonde hair frames a face that always seems on the brink of death. The cylindrical bag on his back holds his weapon. Two braids replace his loose hair after the timeskip.

Belief in fate runs through the whole Blackbeard crew, and for Doc Q it settles on the ideas of fortune and misfortune. He takes note of who seems blessed and who seems doomed, praising the lucky while pitying the rest, yet he will overturn that judgment when circumstances demand, as happened with Luffy. He understands perfectly how frail his body is, accepts the fact, and even finds it worth a laugh.

A terminal illness has never dulled his fighting strength, and earning a place in Blackbeard's crew, which once rejected even a Super Rookie like Jewelry Bonney, speaks to how dangerous he is. Serving as the crew's physician, he treats their injuries, and his grasp of chemistry produced explosive mixtures hidden inside apples that burst on the first bite, which he offers to people who suspect nothing. He kept pace with the titanically strong Burgess while rowing Blackbeard's raft, and at Marineford he tore into a weakened Whitebeard with his double-bladed scythe, swinging it one-handed in swift succession. His endurance borders on the absurd: he outlasted Magellan's venom until an antidote arrived and shrugged off Sengoku's Buddha-form shockwave. He went on to consume a Paramecia-class Devil Fruit, the Shiku Shiku no Mi, which grants him the power to make others sick, and he turns his explosive apples into combat tools.
