Goku and Vegeta battle giant worms and navigate deadly traps inside Super Buu's body before reaching his mind. There, they fight illusions of Gohan, Gotenks, and Piccolo created by Buu's thoughts. When Buu's hunger shifts his focus to desserts, the illusions transform into cakes, and the two Saiyans finally discover their friends sealed in strange pods.
The digestive enzyme attack from the previous episode intensifies as a massive roundworm emerges from the depths of Buu's body. With Goku buried under green slime and the worm closing in, Vegeta shouts at him to fight back. Goku unleashes a burst of energy that destroys the creature and frees himself, though the blast causes Buu to clutch his stomach in pain from the outside.
A second worm arrives, followed by their enormous father. Vegeta's fear of the creatures provides unexpected comedy as the Saiyan prince hides behind Goku. The father worm turns out to be reasonable, apologizing for his sons' aggressive behavior and pointing the Saiyans in the right direction. Outside, Super Buu creates a restroom and uses it, producing a stench so powerful that Dende and Mr. Satan both collapse.
Goku and Vegeta ascend into Super Buu's head, where the terrain shifts from organic grotesqueness to something more surreal. They split up and each encounter what appears to be their loved ones: Goku faces Gohan, while Vegeta confronts Gotenks and Piccolo. The fights feel real and the opponents are dangerously powerful.
But something is wrong. These fighters do not react like the real versions. They are thought forms generated by Super Buu's mind, constructs designed to overwhelm intruders. Goku and Vegeta struggle against the illusions, unable to put them down permanently. Just when the three fakes close in for the kill, Buu's attention wanders to his craving for sweets. The thought forms of Gohan, Gotenks, and Piccolo morph into cakes and pastries, dissolving the threat instantly.
With the illusions gone, Goku and Vegeta press deeper into Super Buu's mind and finally discover what they came for. Gohan, Goten, Trunks, and Piccolo hang suspended in strange organic pods, unconscious and feeding their power directly into Buu. The sight confirms what they feared: their friends are alive but trapped, serving as living batteries for the villain's strength.
They cut the pods down carefully. Goku proposes a plan to escape, but Vegeta flatly refuses to participate. Episode 274 is almost entirely original to the anime, expanding the interior journey with creative obstacles and character moments that deepen the bond between two rivals forced to cooperate in the strangest environment imaginable.
